Forms for Participation in Title VI Funding
ED 506 forms are due Wednesday, March 5.
Title VI funding for PSD is based on the number of ED 506 forms (Indian Student Eligibility Certification Forms) we collect. If your student does not qualify to fill out an ED 506 form, please complete the Native American/Indigenous Student Information form to help us plan initiatives accurately. All families are encouragedd to submit this form.
Who will see my forms?
Your forms stay within PSD and are not shared beyond the district. For grants, we only report the number of verified ED 506 forms. If audited, officials confirm our count but do not receive copies of the forms.
Parent/Caregiver Committee
If you would like to join the Title VI Parent/Caregiver Committee, please complete this Google form. The committee will create bylaws, collaborate on grant goals, and approve the Title VI grant application.
