Transition Pathways (18-21 Services) for students are designed within an outcome-oriented process, which promote movement from school to post-school activities, including post-secondary education, vocational training, employment, adult services, independent living, or community participation.

Activities are based on students' needs taking into account preferences and interests, as well as instruction, community experiences, the development of employment and other post-school adult living objectives. When appropriate, this may also include acquisition of daily living skills and a functional vocational evaluation.

Services are based on current assessments of the student's academic, vocational, and daily living skills. The student's language and culture are considered in evaluations and planning. Transition services emphasize skills needed to live and work in an integrated community setting.
