A Comprehensive Benefits Package

Poudre School District is committed to providing an environment that promotes a healthy employee population able to serve our students at the highest level. We offer a comprehensive benefits package that includes health, dental and life insurance as well as other programs for our eligible employees.
Our benefit package offers you choices of benefits. Your benefits can play a significant role in your everyday life, especially when you understand their value and scope. You will want to take advantage of the benefits available to you.
Enrollment process
The new employee enrollment process for eligible employees will guide you through selecting and signing up for benefits. Please follow the steps below to view your benefits options and complete your online enrollment.
Note: If you enroll in benefits after the 15th of the month in which your benefits are effective, you will see a double deduction in premiums on your following month paycheck.
Step one:
Find the eligibility notice e-mailed to your PSD email explaining when your benefits will be effective (which is the 1st day of the calendar month following the date you meet the eligibility requirements) and what benefits you will be eligible for.
Step two:

Step three:
Register and enroll for benefits online (how to enroll instructions)

Log-on to Simplified Benefits Administrators website to enroll.

On the right side of the screen click on the Medical Plan Members button

Answer Yes to the question "Are you a Poudre School District Employee?"

Click on umaclientservices.org

If you are a new user, click on "New User Registration". The User Type will be "Employee." The Group Number is PSD0000 (PSD then 4 zeroes), then enter your Social Security Number and select Next. If you have previously used the website, enter your User ID and Password then Submit. If you have forgotten your user name and password or have been locked out, please call Simplified Benefits Administrator's at 1-866-644-7873 or 1-800-207-1018. Skip to number 8

Setup user name (any combination of letters and number) and password (case sensitive and must be at least 6 characters) on the User Registration page. Complete page and select Next.

After registration is successful, click on the "Next" button, located at the bottom of the page or at the upper left had side, click on Enrollment and select "Change My Enrollment".

Enter the Corporate Code that was included on the Eligibility Notice that was e-mailed to you by Benefits Services.

For the reason of change, please select "New Enrollment or Change My Enrollment", select Next.

Enter demographic information on New Enrollment Information screen if form is not pre-filled.

Select Next.

Select coverage for medical, dental, and vision on the Plan Information screen, select Next.

Enter dependent information, if applicable. If you are adding both a spouse and child, enter spouse first then you will have the option to enter additional dependents. Social Security numbers for each dependent added are required. When done, select Next.

Enter Dependent Elections, select Next.

Enter whether you want to participate in the medical and dependent care flexible spending accounts, select Next.

Verify information. If correct, print the summary page as your confirmation and select Submit. If you need to change information, you can select Back. If you select Cancel, all information will be erased and you will need to start again.

If you enrolled any dependents to your medical, dental or vision plans, you will be required to submit proof of relationship documents to Benefits Services before your enrollment is approved. A list of acceptable proof of relationship documents is attached to the Eligibility Notice that was e-mailed to you by Benefits Services. Email the necessary documents to PSDbenefits@psdschools.org.
- Note: Your online enrollment must be submitted within 31 days of your benefits effective date.
Step four:
Check your mail for your benefits cards if applicable.
Medical Cards - Will be mailed to your home address from Simplified Benefits Administrators within 7-10 business days from when you submit your online enrollment. Replacement cards must be requested from Simplified Benefits Administrators at 1-866-644-7873 or 1-800-207-1018.
Dental - Delta Dental does not send physical cards in the mail. Please visit deltadentalco.com and register to print out a card. You will not receive cards in the mail. You must download the mobile app AND/OR create an account online (using your SSN as the member ID).
Prescription Cards - Will be mailed to your home address from OptumRx. within 15 days from when you submit your online enrollment. Replacement cards must be requested from OptumRx at 800-880-1188.
Vision - Vision Service Plan does not issue vision cards.
Step five:
- Enroll in the Optional Legal Service Plan offered by ARAG (this voluntary benefit is optional and not required). Please visit their website to enroll.
*Your online enrollment for the Optional Legal Service Plan must be submitted within 31 days of your benefits effective date. Otherwise you are not eligible to enroll in these optional benefits until the next open enrollment period.

Complete the Voya's Optional Life AD&D Enrollment Form if you are choosing to elect optional coverage for you and your eligible family members. Send completed form to the PSD Benefits Services Department (this voluntary benefit is optional and not required).

Complete the Voya's Beneficiary Designation Form for the $50,000 basic life insurance policy provide to you by PSD. Send completed form to the PSD Benefits Services Department.
Benefits Administration Contacts
Health Benefits - PSD's health benefits are self-funded and administered by Simplified Benefits Administrators. If you have questions regarding a claim status, deductibles, FSA questions and or need to change your PCP, please contact Simplified Benefits Administrators at 1-866-644-7873 or 1-800-207-1018. Dental Benefits - Our dental benefits are administered by Delta Dental. If you have any questions regarding a claim status or your annual out-of-pocket maximum, please contact Delta Dental by visiting their website deltadentalco.com. There is a contact us section that can guide you to the correct department for your needs. To find an in-network provider, please also visit deltadentalco.com. You will not receive cards in the mail. You must download the mobile app AND/OR create an account online (using your SSN as the member ID).
Prescription Benefits - Our prescription benefits are offered through OptumRx. If you have any questions about the cost of a prescription and or your annual out-of-pocket maximum, please contact OptumRx at 1-800-880-1188.
Vision Benefits - Vision benefits are offered through Vision Service plan. If you have any questions on how the coverage works, co-payments, claim status or reimbursements, please contact Vision Service Plan at 1-800-877-7195. Access a list of providers on VSP's website.
Mental Health Benefits - PSD's mental health benefits are self-funded and administered by Employee Assistance Services (EAS). If you have questions regarding EAS mental health benefits, community mental health network providers, referral authorizations, copay/coinsurance, deductibles, mental health out-of-pocket maximums, etc., please contact EAS at (970) 488-4925 or visit their website at eas.psdschools.org.
