Newly Eligible Employees

Poudre School District is committed to providing an environment that promotes a healthy employee population able to serve our students at the highest level. We offer a comprehensive benefits package that includes health, dental and life insurance as well as other programs for our eligible employees.

Our benefit package offers you choices of benefits. Your benefits can play a significant role in your everyday life, especially when you understand their value and scope. You will want to take advantage of the benefits available to you.

Enrollment Process

An eligibility notice will be mailed to you explaining when your benefits will be effective and what benefits you will be eligible for.

Please review this Employee Benefits Packet for information about the benefits offered through Poudre School District. Below are the steps to reigister and enroll online.

  1. Log-on to United Medical Alliance's website to enroll at www.unitedmedical.org
  2. Under "Client Login" click on "Members"
  3. If you are a new employee, click on the "Register" button. If you have already enrolled on United Medical Alliance's website enter the user name and password you created for their site.
  4. Select a User Type (Employee) and enter Group Number (PSD0000) - This step is only for new employee's that have not yet registered on UMA's website.
  5. Continue by completing additional fields until the registration process is complete and you arrive at the welcome page.  You have the option to print the enrollment tutorial located on this page to assist you through the benefits enrollment process.
  6. To start the enrollment process, click on the "Next" button, located at the bottom of the page.
  7. Enter the Corporate Code that was checked on the Eligibility Notice that was mailed to you by Benefits Services.
  8. Complete all required fields on the following pages until your enrollment is complete.

Your online enrollment must be submitted within 31 days of your benefits effective date.

Benefits Cards

Medical Card - Will be mailed to your home address from United Medical Alliance (UMA) within 7-10 business days from when you submit your Health and Dental Enrollment Form to the Benefits Office. Replacement cards must be requested from UMA at (970) 224-4600.

Dental Card - Will be available at all benefits orientations and or in the benefits office. Replacement cards may be requested from the Benefits Office.

Prescription Card - Will be mailed to your home address from Medco Health Solutions within 15 days from when you submit your forms. Replacement cards must be requested from Medco at 800-711-0917, press 0.

Vision Card - Vision Service Plan does not issue vision cards.

Benefits Administration

Health Benefits - Poudre School District's health benefits are self-funded. Our health benefits are administered by United Medical Alliance. If you have questions regarding a claim status or deductibles, please contact United Medical Alliance at (970) 224-4600. You may also access a list of providers on United Medical Alliance's website.  For directions on how to locate a doctor on UMA's website, please click here.

Dental Benefits - Our dental benefits are administered by MetLife. If you have any questions regarding a claim status or your annual out-of-pocket maximum, please contact MetLife at 1-800-942-0854. You may also access a list of dental providers on MetLife's website.

Prescription Benefits - Our prescription benefits are offered through Medco Health Solutions. If you have any questions about the cost of a prescription and or your annual out-of-pocket maximum, please contact Medco at 1-800-711-0917, press 0.

Vision Benefits - Vision benefits are offered through Vision Service plan. If you have any questions on how the coverage works, co-payments, claim status or reimbursements, please contact Vision Service Plan at 1-800-877-7195.  You may also access a list of providers on VSP's website.
